LIFELINE ASSISTANCE

Lifeline

Phone and Internet Assistance Program

 

People across the United States rely on phone and internet to stay connected to family, work, education, and essential services. The Lifeline Program provides a monthly benefit to reduce the cost of these connectivity services for qualified households.

 

 

Program Rules:

  • Lifeline is available only to subscribers who can document their eligibility.

  • Only one Lifeline benefit is permitted per household.

  • Subscribers are required to recertify their eligibility every year.

How It Works:

Lifeline offers a monthly benefit to eligible subscribers on either home phone ($7.25)
or home internet ($9.25) service.

 

 

Eligibility:

Consumers are eligible for a Lifeline benefit if they are currently enrolled in one of the
following programs:

  • Medicaid

  • SNAP (Supplemental Nutrition Assistance Program) or Food Stamps

  • Supplemental Security Income (SSI)

  • Federal Public Housing Assistance

  • Veterans Pension and Survivors Benefit

  • Tribal Specific Programs

  • Income-based eligibility (at or below 135% of the federal poverty guidelines)

 

You must show a card, letter, or official document, as proof that you participate in one
of these programs when you apply for Lifeline. For proof of income please provide
three consecutive pay stubs or a tax return.

 

Annual Recertification:

Subscribers will be recertified either automatically through the National Verifier's state/federal data sources, or receive a recertification request from USAC.

APPLY ONLINE:

Consumers can sign into this site from any computer or mobile device to create an account, receive an eligibility decision, and use the list of service providers in their area to contact one to enroll.

APPLY BY MAIL:

Consumers can mail a paper application and copies of proof of eligibility to the Lifeline Support Center, which will return an eligibility decision by mail in 7-10 days.

 

Then, the customer will contact a service provider to enroll.

 

The applicant must complete all fields fully and correctly, initial each agreement, and sign the form. Note: All fields are required unless otherwise indicated.
